• Identification and Insurance: Please remember to bring your valid identification and insurance card with you to your appointment. These documents are essential for proper registration and to facilitate a seamless check-in process. 
• Pre-Appointment Forms: As a new patient, you will be required to complete the new patient package, which includes important medical history and consent forms. There are three ways to obtain these forms: 
  • Email: We can email the forms to you prior to your appointment. Please let us know if you prefer this option.
  • Receptionist Assistance: If you prefer to complete the forms on the day of your visit, our friendly receptionist will be happy to assist you. Please arrive 15 minutes prior to your scheduled appointment to allow ample time.
  • Self-Service: If you are aware that you are a new patient, you can simply pick up the new patient package from the designated area next to the sign-in sheet upon your arrival.
• Preparation and Questions: We encourage you to come prepared with any questions or concerns you may have for the provider. Dr. K. Alex Kim and our knowledgeable team are dedicated to addressing all your inquiries and ensuring that you have a clear understanding of your reconstructive surgery. 
• Journey Guidance: Following your consultation with Dr. Kim, you will be introduced to Veronica, who will be your guide throughout your procedure journey. She will provide you with valuable information, support, and assistance to help you navigate every step of the way. Additionally, Arianna is also available to offer any further assistance you may require. 
• Punctuality: We kindly request that you arrive on time for your scheduled appointment. Being punctual allows us to provide the attention and care you deserve. If you are more than 15 minutes late, it may be necessary to reschedule your appointment to ensure proper allocation of time and resources. 
• Active Contact Information: To ensure effective communication, please provide us with your current and active phone number and email address. This allows us to share important updates, appointment reminders, and any changes to your scheduled visits. Rest assured that we uphold strict confidentiality regarding your personal information.
