Eyelid Surgery for Under-Eye Bags

A subciliary incision is made

To begin this surgical procedure, a precise cut is carefully made just beneath the lower lashes to start the eyelid surgery. This location not only helps to hide the incision but also provides clear access to the important area beneath the eyes where correction is needed.

Suspending the lid margin

After suspending the lid margin with a suture to protect the globe, lower eyelid skin muscle flap is being elevated.
The lower eyelid is gently supported with a suture to protect the eye. The skin and muscle of the lower eyelid are lifted together to reveal the area underneath.

Excess fat is removed

Under the skin muscle flap is thin fascia of orbital septum. Once this is opened, periorbital fat is exposed. These excess fat is responsible for the common under-eye puffiness, or “bags” under the eyes. This is when Excess fat is removed with electrocautery.

Medial fat is being removed

The fat located near the inner corner of the eye is removed first. This helps smooth out the puffiness that appears in that area.

Central fat is being removed

Next, the fat in the middle part of the lower eyelid is taken out. This area is often the most prominent part of the under-eye bag.

Lateral fat is being removed

Finally, the fat on the outer part of the eyelid is removed to complete the contouring. This helps create a more even and refreshed look.

Hemostasis is achieved

Once the fat is removed, the area is checked to stop any bleeding. This helps ensure a safe and clean healing process.

Incision is closed

Any loose skin that remains is trimmed for a smoother result. The incision is then closed carefully to leave a minimal scar.

The skin removal step is finished and the eyelid is reshaped. This helps create a more lifted and open appearance. The new contour enhances the natural eye shape, giving the face a brighter and more refreshed look.

Hemostasis with electrocautery

Any minor bleeding is carefully controlled using a special tool designed to seal blood vessels during the procedure. This keeps the surgical area clean and visible for the surgeon but also significantly reduces the risk of bruising and swelling after surgery, promoting a smoother recovery.

Medial fat is being removed

Dissection was extended deep to the muscle and orbital septum, entering the periorbital space to find excess fat in the area. The area under the muscle is carefully opened to reach more fat that may be causing puffiness. Fat near the inner corner of the eyelid is removed for a smoother appearance.

Upon removal of the fat

After the fat is carefully removed, the area is thoroughly examined to ensure that the contours of the eyelid are smooth and symmetrical. This step is essential to create a balanced, natural-looking result that appears even on both eyes and enhances the overall appearance.

The incision is closed with sutures

The cut is closed with stitches that help the skin heal neatly. This final step completes the eyelid surgery and leaves a more refreshed, alert look.

My dear friend, Dr. Hinika, a general surgeon based in Los Angeles and originally from Ethiopia, identified an urgent need for a hospital in his hometown. Motivated by his commitment to serving his community, he established the Negele Arci General Hospital and Medical College in 2018, bridging a critical healthcare gap in a region that previously lacked a hospital within a 50-mile radius.

I had the privilege of attending the hospital’s inauguration, where I contributed by performing reconstructive surgeries. This year marked a significant milestone as the institution celebrated the graduation of its inaugural class of medical students. I was honored to participate in the festivities, and I also took the opportunity to perform surgeries and share my expertise with the graduating students.
